Starting the Optometry Hypertension Case Finding service

The below guide shows how to initiate an optometry hypertension case finding service appointment for a patient.

1) The process begins in the patient management screen where the first booking can be made. Having found the patient on the database, you will have the ability to select the enhanced cataract referral tab, adjacent to the patient’s name.

Ensure you have the correct patient and select any patient factors required.

2) Next, collect the patients preferred contact number. Record if they (or their partner) are in receipt of any income related benefits. Recording of ethnicity and if the patient has a disability they wish to disclose is required to be recorded. This is especially important as in some areas, the service is targeting patients in deprived areas. Capturing this data allows the service to evidence this.

4) The next page allows for eligibility of the service to be confirmed, as well as consent for the ability for their record to be held and consent for communication with them via SMS, email, letter or phone. As well as the patient’s explicit consent to view their summary care record.

Carrying out the Optometry Hypertension Case Finding

Following this, you then record the measurement details. 

The date of assessment, if a reading was possible and if so, what the systolic and diastolic readings were – the lower of at least 2 measurements should be entered onto OPERA. Also record if an irregular heartbeat was detected. Following that, the measurement outcome is selected from the drop-down list:

Please note, that this is selected by the colleague completing the episode entry on OPERA. It is not extracted from the measurements recorded or other data inputted. Depending on which ‘Measurement Outcome’ is selected there will be guidance on screen of where the patient should be directed to. This is open to local variation, and you should follow the guidance for your specific area. 

Following this on the next page, comments to be added to the GP notification and what healthy living advice has been provided can be recorded. The method of the patient outcome being shared with the patient can be selected – via email or print in practice for a hard copy.

The opt in/out for patient feedback (PROMS) is also on this screen. We ask that as many patients as possible are asked to contribute to PROMS, as this is vital evidence for the continuation of the service to be commissioned, especially if it is a pilot phase. 

On the final screen the outcome is confirmed. Please note, this is populated from the ‘Measurement Outcome’ from the earlier screen. It cannot be changed on this screen. If it needs to be amended, please click the ‘Back’ button until you get to the relevant screen. 

The episode is completed by clicking on the ‘Submit Assessment’ button.
