Guidance for practices when Closing an Optical Practice and terminating the subcontract with Primary Eyecare Services.

Step 1:  Notify Primary Eyecare Services providing at least 6 months written notice of their intentions of the closure.

The current subcontractor must adhere to the termination clause (section 7.5) of the Practice Subcontract with Primary Eyecare Services, “The Subcontractor may bring the Agreement to an end at any time by issuing the Co-ordinating Provider (PES) with not less than6 months’ written notice to that effect”).  Therefore, the Subcontractor is obligated to inform Primary Eyecare Services in writing about their intentions regarding their current subcontract and the services they provide.

Step 2:  Inform Primary Eyecare Services of your intention to Close the practice and Complete the Practice Closure Notification Form and return to hello@referral.support. 

The Current Subcontractor holder is responsible for notifying PES of the intention to close the practice and what process has been decided with regards to practice records by completing the Practice Closure Notification form available HERE and please return it to hello@referral.support

Step 3: PES Team will acknowledge the practice intentions and process the change.

The PES Team will process the change in accordance with the details submitted on the Practice Closure Notification form and confirm if they require any further information and when the process has been completed.

Please Note:

  • If the practice closes, the practice owner should: arrange to transfer the patient records to another registered practitioner or practice and inform all patients that this process will take place and the intended date.
  • The practice owner would be required to report the practice intention to end the General Ophthalmic Services (GOS) contract, please contact the NHS Business Service Authority via pao-cm@nhsbsa.nhs.uk
