For the Gloucestershire Low Vision Service, any patients requiring Low Vision Aids ordering will need to complete the form here:

Low Vision Zero VAT Eligibility Form 

This needs to be completed and then sent to lgmch.peslv@nhs.net before Low Vision Aids can be ordered for patients.
