Low Vision Service Zero VAT Declaration Form
For the Low Vision Service, any patients requiring Low Vision Aids ordering will need to complete the form found here: Low Vision Zero VAT Eligibility Form. Before Low Vision Aids can be ordered for patients, the Low Vision Zero VAT Eligibility Form must be completed, and the appropriate process followed based on the patient's location, as stated below.
Lancashire and South Cumbria: The completed form must be uploaded to the patient's episode and sales@assopt.co.uk must be entered in the 'LVA Provider Email' field.
- Gloucestershire: The completed form must be uploaded to the patient's episode and lgmch.peslv@nhs.net must be entered in the 'LVA Provider Email' field.
