Completing PROMS where a patient has asked for support

Patient feedback is an important part of any service, which is why after completion of a service episode, we would ask all practitioners to encourage patients to give feedback. This has been implemented into all modules on OPERA and the patient can be sent either a text or an email, containing a link to a short survey.

Some patients may consent to give service feedback for PROMS (Patient Reported Outcome Measures) but need support from your practice to complete. They may not have an email address or a mobile phone number or may just prefer to complete a paper form. You can support by following the steps below.

1)    Select “Send PROMS to patient by email” and in the text box, type in your optical practice’s email address:

2)    Provide the patient with a printout of the PROMs questionnaire relevant to the service Paper-PROMS-form.pdf. For CUES, MECs and CES services, there is the extra question of where the patient would have attended if the service had not been available Paper-PROMS-form-CUES-MECS-CES.pdf. For all other services such as Post Cataract, Children’s Services etc this question is not included.

3)    Once the patient has completed the questionnaire, you can then access your practice email, find the relevant email from Optometry Referrals and click the link to fill in these boxes on behalf of the patient by copying their answers on the paper form. This does not have to be completed by the clinician and could be delegated to another staff member where appropriate.
