This service is delivered with recording via the CES pathway function on Opera. With patients seen within 24/48hrs for urgent patients or 2weeks for non-urgent patients. 

Triage/Screening Form for non-clinical staff that can be used can be found here.         

Patients can self refer into the service or will be referred to the practice via the Single Point of Access (SPA) following triage of routine referrals from the PES Referral Hub.

The Moorfields Croydon SPA Triage Criteria can be found here.

Patients may be referred into the service for same-day appointments following redirection from the Moorfields Croydon Rapid Access Clinic, as appropriate.
