Completing a face to face assessment (Core) with an onwards referral to HES

This video shows how to complete the face to face assessment form (also known as Core assessment within the application) and then send the referral to a Hospital service.  You should also follow your PES CUES SOP for further information regarding onwards referrals to HES.

You can refer to various types of provider from within OPERA - these include HES, OCT practices, Prescriber Practices and also other CUES practices if you are unable to meet SLA targets for seeing patients.
